Metro 2039 will bring back the hidden Karma system from previous games, with 4A Games changing how it works to fit the new story.
Executive producer Jon Bloch confirmed the returning mechanic in an interview with GamesRadar, saying the idea of actions having consequences remains part of the series. 4A has not revealed which decisions will affect Karma in Metro 2039 or how those choices will influence the story.

Previous Metro games quietly awarded Karma Points based on decisions, exploration, conversations, and how players approached certain encounters. The system operated largely in the background and could contribute to which ending players received.
Bloch said 4A has handled Karma differently in each game and will adjust it again for Metro 2039. The studio wants the system to serve the new story rather than simply repeat how it worked in Metro 2033, Last Light, or Exodus.
4A is keeping the exact rules under wraps for now. Bloch did not confirm what will earn positive or negative Karma for the Stranger, leaving players to discover the consequences of their choices when the game launches.

While Karma is returning, 4A is taking a different approach to the morality surrounding the main conflict.
Bloch said previous Metro games dealt heavily with moral gray areas, while Metro 2039 gives the conflict a better moral position. The story takes place under the Novoreich, an authoritarian regime led by Hunter that controls the Metro through propaganda and fear while promising a new war for the surface.
The studio has said its experience of Russia's invasion of Ukraine influenced the direction of the game. Metro 2039 focuses on the consequences of war and authoritarian rule while still allowing individual actions to carry consequences through the Karma system.
Metro 2039 introduces a new voiced protagonist known as the Stranger. Living in exile outside Moscow, he is forced to return to the Metro after violent waking nightmares bring his past back into his life.
The game is set six years after Metro 2033 and is being written in collaboration with series author Dmitry Glukhovsky. 4A is returning to a story-driven single-player campaign built around exploration, survival, combat, stealth, and psychological horror.
Metro 2039 is scheduled to launch in February 2027 for PS5, Xbox Series X|S, and PC.
