Mafia: The Old Country is a third-person action-adventure game set in early 1900s Sicily. It follows Enzo Favara, a former forced laborer who risks everything to join the Torrisi crime family and build a better life. The campaign uses a linear, narrative-driven structure as Enzo becomes increasingly involved in Sicily’s criminal underworld. Missions combine gunfights, knife combat, stealth, and exploration, with period weapons including pistols, rifles, shotguns, and stilettos. Some encounters allow players to choose between stealth and direct combat, while others require a specific approach for story reasons. Enzo travels through towns, vineyards, ruins, countryside, and other Sicilian locations on foot, horseback, and in early automobiles. Knives have different properties and can be used for melee attacks, silent takedowns, lockpicking, and other actions, while firearms provide ranged options during larger fights. Players can also collect Dinari to purchase weapons, vehicles, horses, outfits, and other equipment. Although the campaign remains the main focus, Free Ride mode allows players to explore Valle Dorata more freely and take on separate combat and racing challenges.
